Responsibilities

• Assess, monitor, and document patient symptoms.
• Collaborate with patient/family and interdisciplinary team to develop care plans.
• Advise on patient rights, reimbursement procedures, and education needs for caregivers.
• Observe and evaluate patient status and safety factors to determine education needs for caregivers.
• Supervise and direct aides and licensed practical nurses.
• Participate in on-call schedule and provide backup assistance as needed.
