Job summary

Paradigm Health in Indiana is seeking a Full-time Hospice CNA to join a patient-focused team. Schedule: Monday through Friday, 8a-5p; territory openings are assigned near the office this posting is linked to.

Provide compassionate care, assist with activities of daily living, document vital signs in EMR/EHR, and collaborate with the care team while maintaining HIPAA compliance. Join a Top Workplace with strong PTO and benefits.

Qualifications

• 1 years of CNA experience required.
• Licensed CNA in State of Indiana.
• One year Hospice experience preferred.
• Current driver's license.
• Must have reliable transportation and insurance.

Responsibilities

• Provide compassionate care to hospice patients, ensuring their comfort and dignity.
• Assist with daily living activities such as bathing, dressing, and grooming.
• Monitor and document patient vital signs and changes in condition using EMR/EHR systems.
• Collaborate with healthcare team members to develop and implement care plans.
• Adhere to HIPAA regulations and maintain confidentiality of patient information.
• Support families by providing emotional support and education about hospice services.
• Drive to patient locations as needed, ensuring timely and reliable service.

• Available territory openings: CNAs usually assigned to 1-3 counties near the office this position is posted to
• Provide compassionate care to hospice patients, ensuring their comfort and dignity.
