Job summary

Paradigm Health in Indiana is seeking a Full-time Hospice CNA to join our patient-focused team. This role involves providing compassionate care, assisting with daily living activities, and documenting patient status using EMR/EHR systems.

The ideal candidate will have a valid Indiana CNA license, at least 1 year of CNA experience, and a preference for hospice care. A current driver’s license and reliable transportation are required.

Qualifications

• 1 year CNA experience required.
• Licensed CNA in State of Indiana.
• One year Hospice experience preferred.
• Current driver's license.
• Must have reliable transportation and insurance.

Responsibilities

• Provide compassionate care to hospice patients, ensuring their comfort and dignity.
• Assist with daily living activities such as bathing, dressing, and grooming.
• Monitor and document patient vital signs and changes in condition using EMR/EHR systems.
• Collaborate with healthcare team members to develop and implement care plans.
• Adhere to HIPAA regulations and maintain confidentiality of patient information.
• Support families by providing emotional support and education about hospice services.
• Drive to patient locations as needed, ensuring timely and reliable service.

Job Description

• Full-time Hospice CNA
• Schedule: Monday thru Friday, 8a-5p
• Available territory openings: CNAs usually assigned to 1-3 counties near the office this position is posted to
