Job summary

Ascend Hospice & Palliative Care MA is seeking an RN On Call Nurse for weekend shifts. Starting salary is $93,600 per year. Responsibilities include comprehensive nursing care, pain and symptom management, and coordination with physicians and the care team.

Qualifications

• Must have State RN license.
• Experience in Hospice or home health care is REQUIRED.
• Excellent documentation skills and ability to work independently.
• Strong communication skills.
• Valid driver’s license and reliable transportation.
• Must be available to travel locally within assigned territory.

Responsibilities

• Assumes responsibility for a patient visit or on-call schedule including assessment, planning, implementation and evaluation of nursing care.
• Provides hospice nursing care with pain and symptom management.
• Communicates with attending physicians and hospice staff to coordinate care.
• Maintains up-to-date documentation at point of service.
• Administers medications per laws and hospice policies.
• Collaborates with Medical Director to determine eligibility of hospice patient.
• Assesses patient and family to determine home care needs.
• Re-evaluates patient nursing needs and adjusts care plans as needed.
• Counsels patients and families on end-of-life needs.
• Participates in on-call duties as defined by policy.
