Job summary

Capital City Hospice – an Addus family company is seeking a Registered Nurse (RN) for Hospice On Call / Triage in Columbus, OH and surrounding areas. The role involves triaging after-hours patient and family calls, arranging nursing visits, and coordinating with the interdisciplinary team to ensure responsive, compassionate end-of-life care.

The position requires an accredited RN program graduate, an active OH RN license, CPR, and at least two years of nursing experience.

• Current license to practice as RN in the state.
• Minimum of two years of varied professional nursing experience.
• Demonstrated nursing practices and clinical judgment.
• Ability to cope with family emotional stress and diverse lifestyles.

Responsibilities

• Triage after-hours calls, determine needs for services, assess and update care plans.
• Collect data on patients and families to inform care and coordinate with interdisciplinary team.
• Coordinate care by initiating communications with attending physician and other agencies.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate clinical records with timely care plans and progress notes.

Location: Columbus, OH and surrounding areas
Schedule: Triage every other Sat/Sun 8a-8p and 2 12hr extender shifts/week

What You’ll Do:

• Assume responsibility for triage of after-hours patient and family phone calls, determining the need for services including assessments, symptom management and care plan updates, delegating nursing visits to appropriate staff members
• Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team as appropriate
• Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
• Perform clinical pronouncement of death and provide support to family
• Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes

Qualifications:

• Graduate from an accredited registered nursing program
• Possession of a current license to practice in the state as a Registered Nurse
• Minimum of two years of varied work experience as a professional nurse
• Demonstrated skill in nursing practices and clinical judgments
• Ability to cope with family emotional stress and tolerance of individual lifestyles
• Must possess current CPR Certification
• Valid driver’s license and proof of insurance is required
