Job summary

Enhabit Home Health & Hospice is seeking an experienced registered nurse for 7 On/7 Off on-call shifts, coordinating care and implementing the interdisciplinary plan.

You will perform services per physician orders, ensure regulatory compliance, and maintain safe transportation and CPR certification. Qualifications include RN license in the state, at least 1 year RN experience, and basic tech skills.

Qualifications

• Must hold a valid RN license in the state of practice.
• Minimum of 1 year of licensed RN experience.
• Basic demonstrated technology skills, including mobile device use.

Responsibilities

• Coordinate care and assess patient and family needs, implementing the care plan.
• Deliver services per physician orders and the established plan of care (POC).
• Ensure care delivery complies with federal and state regulations.

Responsibilities

7 On/7Off - On Call Wednesday A.M. to Wednesday P.M.

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
