Job summary

Enhabit Home Health & Hospice in Oklahoma is seeking a compassionate Registered Nurse to coordinate patient care in the home setting. You will work within the interdisciplinary care team to assess needs and implement the plan of care, following physician orders and ensuring compliance with federal and state regulations.

Requirements include graduation from an approved school of nursing, an active RN license in the state, at least one year of RN experience, and basic technology skills including

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
