Job summary

Enhabit Home Health & Hospice is seeking an experienced Registered Nurse for the Montgomery, AL area in a 7 On/7 Off on-call rotation. The role focuses on coordinating care, assessing patient needs, and implementing interdisciplinary care plans within a home health environment.

The ideal candidate will hold an RN license, have at least one year of RN experience, and demonstrate proficiency with mobile devices and healthcare technology.

Qualifications

• Must be a graduate of an approved school of nursing and licensed as RN.
• Medicare home health or hospice experience is preferred.

• Coordinate of care, assess needs of patients and families, and implement the interdisciplinary care plan.
• Perform services in accordance with physician orders and the established plan of care (POC).

Responsibilities

7 On/7Off - On Call Wednesday A.M. to Wednesday P.M.

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician's orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
