Job summary

Job Summary

The on-call RN is responsible for coordinating care, providing direct care/support for hospice patients and their families in the home and various nursing facility settings. Flexible schedule, full time, part-time per diem. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed.

Responsibilities and Duties

On-call support after agency hours for hospice patients/families with regard to comfort, symptom management, medication management, DME needs, admissions, discharges, proper documentation and coordination of care between hospice interdisciplinary team & facilities.

Qualifications and Skills
