Full-Time or PRN options available. You will manage patient care, coordinate with the interdisciplinary team, complete admissions and recertifications, educate families, and maintain documentation using a company tablet.

Qualifications

• Active Texas RN license required.
• Strong assessment and communication skills required.
• Ability to work independently in the field.
• Reliable transportation for local travel.

Responsibilities

• Provide hospice nursing visits to patients in their homes or care facilities.
• Manage patient care and coordinate with the interdisciplinary hospice team.
• Complete routine visits, admissions, and recertifications.
• Educate and support patients and families during end-of-life care.
• Maintain documentation using a company-provided tablet.
