Grace Healthcare Services in Newark, NJ is seeking a Hospice Registered Nurse (RN) to join our Interdisciplinary Team. You will assess and address patients' needs, initiate interventions on admission, and maintain a continuous plan of care throughout the patient journey.

The role requires a valid NJ or multi-state RN license and at least two years of acute-care nursing, with strong communication and independent work ability. Travel to patient locations is required.

Qualifications

• Valid New Jersey or Multi-State RN license.
• Minimum of two (2) years of nursing experience in acute-care settings such as medical, surgical, or oncology units.
• Strong clinical practice with customer service, excellent communication abilities, and the capacity to work independently in a fast-paced environment.

Responsibilities

• Deliver skilled nursing care as outlined in individualized care plans, ensuring comfort and quality of life for terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans as needed.
• Educate and support patients' families and primary caregivers, providing guidance on care techniques and offering emotional support during this challenging time.
• Serve as a liaison between patients, families, and other healthcare providers, ensuring clear communication and advocating for patients' wishes and needs.
• Travel daily to patient locations and facilities as assigned.
