Job summary

Grace Healthcare Services in New Jersey seeks a compassionate Hospice Registered Nurse (RN) to join our Interdisciplinary Team. You will assess patient and family needs, initiate interventions on admission, and maintain a continuous, responsive plan of care throughout the journey.

Responsibilities include delivering skilled nursing per care plans, collaborating with physicians, social workers, and chaplains, educating families, and advocating for patients' wishes.

Qualifications

• Valid New Jersey or Multi-State RN license.
• Minimum of two years of nursing experience in acute-care settings (medical, surgical, or oncology).
• Excellent communication abilities and strong customer service orientation.

Responsibilities

• Deliver skilled nursing care per individualized care plans to terminally ill patients.
• Collaborate with IDT including physicians, social workers, chaplains, nursing assistants, and other stakeholders to adjust patient care plans.

• Travel daily to patient locations and facilities as assigned.

Role Overview

As a Registered Nurse (RN) at Grace Healthcare Services, you will be an integral member of our Interdisciplinary Team (IDT), responsible for assessing and addressing the comprehensive needs of patients and their families. Your role involves initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care throughout the patient's journey.

Key Responsibilities

Deliver skilled nursing care as outlined in individualized care plans, ensuring comfort and quality of life for terminally ill patients.

Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans as needed.

Serve as a liaison between patients, families, and other healthcare providers, ensuring clear communication and advocating for patients' wishes and needs.

• Travel daily to patient locations and facilities as assigned.

Qualifications

Valid New Jersey or Multi-State Registered Nurse (RN) license.

Minimum of two (2) years of nursing experience in acute-care settings such as medical, surgical, or oncology units.

Strong clinical practice with a focus on customer service, excellent communication abilities, and the capacity to work independently in a fast-paced environment.
