Job summary

Trinity Hospice 2 LLC is seeking an experienced Hospice nurse to join our team PRN and assist with admissions in the Warrenton, VA area. You will conduct comprehensive patient assessments and collaborate with an interdisciplinary team to develop and implement comfort-focused care plans.

The ideal candidate holds an active RN license in the state, has hospice or RN Case Manager experience, and can provide reliable transportation.

Responsibilities

• Perform comprehensive nursing assessments of patients and families to identify physical, psychosocial, and emotional needs.
• Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort and quality of life.
• Provide skilled nursing interventions, including pain and symptom management, medication administration, wound care, and ongoing clinical monitoring.
• Use clinical judgment to recognize changes in condition and intervene appropriately.
• Serve as the primary RN case manager, coordinating care across the interdisciplinary team.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, and hospice aides to ensure seamless care delivery.
• Facilitate communication among team members to support timely and effective care planning.
• Maintain accurate, timely nursing documentation in accordance with regulatory standards (Wellsky/Consolo).
• Participate actively in interdisciplinary team meetings, care conferences, and ongoing care planning.

A Day in the Life:

Qualifications:

• Current Registered Nurse license in the state which you will be working in.
• One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred.
• Must have a current Driver's license and dependable transportation.
