Concordia Lutheran Ministries is looking for two full-time Hospice RN Liaisons to join the Good Samaritan Hospice team in Pennsylvania. In this role, you will provide clinical assessments to patients and families considering hospice care, ensuring they feel supported and informed during an important decision.

We seek compassionate individuals with a minimum of two years of nursing experience and a valid RN license in Pennsylvania. This is an opportunity to make a meaningful impact while working within a supportive team environment.

Qualifications

• Must have a current RN license in Pennsylvania.
• Minimum of two years of nursing experience required.
• Experience in marketing/public relations preferred.
• Knowledge of hospice and palliative nursing is required.

Responsibilities

• Conduct clinical assessments for patients considering hospice care.
• Build relationships with hospitals and community providers.
• Assist with admissions when needed.
