Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Must have an active RN license in the state served.
• Minimum of 1 year of nursing experience required
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Two years of hospital / hospice / homecare RN nursing experience preferred.

Responsibilities

• Provide compassionate, high-quality hospice nursing care in the patient's home environment.
• Develop, implement, and adjust comprehensive care plans.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Educate patients and families, offering teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
• Utilize AI-powered clinical documentation and best practices to optimize care delivery.
