Job summary

CommonSpirit Health at Home is seeking a compassionate Hospice RN to deliver expert nursing care in patients' homes. You will lead care plans, collaborate with a multidisciplinary team, and support families through the healing journey.

This role emphasizes autonomy, ongoing education, and the use of technology to document care and improve outcomes. A state RN license, CPR/BLS, and a valid driver's license are required.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and a patient-centered approach.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient’s home.
• Craft individualized care plans and adjust them to promote independence and well-being.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
• Utilize technology to optimize care delivery and maintain precise documentation.
