Job summary

Three Oaks Hospice is seeking a compassionate Registered Nurse to provide skilled nursing care in patients' homes across Rockmart, Cedartown, and Rome. You will coordinate with the interdisciplinary team and families to deliver high-quality end-of-life care.

The position is full-time, daytime hours, with on-call responsibilities as needed. Applicants should hold an associate or bachelor's degree in nursing, an active RN license, CPR certification, and strong communication skills.

Qualifications

• Graduate with an Associate or Bachelor's degree in Nursing.
• Current RN license in the state and a valid driver's license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; MS Office familiarity preferred.
• Strong interpersonal, communication and organizational skills.

Responsibilities

• Assess patients and families to determine need for hospice care and plan care.
• Coordinate care with the interdisciplinary team and physicians.
• Educate patient/family about disease, care plan, and end‑of‑life options.
• Provide on‑call nursing coverage as assigned.
• Document care and progress notes; monitor outcomes and adjust plan.
• Support and empower families navigating hospice services.
