Job summary

Three Oaks Hospice is seeking a compassionate Registered Nurse to provide skilled nursing care in patients’ homes across Gwinnett, Forsyth, and Dekalb Counties. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure seamless care.

Work schedule: Friday 5pm to Monday 9am, with ongoing support for end-of-life care. Requirements include an active RN license in the state, CPR certification, and at least two years of clinical nursing experience in

Qualifications

Graduate of an accredited Associate or bachelor’s degree in Nursing.
Current state RN license and valid driver’s license.
Two years’ RN clinical care experience; hospice/home health preferred.
Current CPR certification.
Proficient with computer use; MS Office experience preferred.
Strong interpersonal skills and ability to support patients and families.
Ability to cope with family emotional stress and diverse lifestyles.

Responsibilities

Administer skilled nursing care for clients in their homes.
Coordinate care with the interdisciplinary team, patient, family, and referring agency.
Assist in coordination of care across services and documentation.
Provide education and support to patients and families as part of care planning.
