Qualifications

• Associate or Baccalaureate School of Nursing graduate.

• Two years RN experience in clinical care preferred, hospice/home health ideal.
• Strong communication, interpersonal skills; ability to handle grief concerns.
• Reliable transportation and CPR/BLS compliant.

Responsibilities

• Provide nursing care to clients in their residence and coordinate with IDG.
• Promote hospice mission, goals, safety, and privacy protections.
• Accept on-call duties and participate in required education.
• Conduct admissions, eligibility assessments, and plan of care updates.
• Educate patients and families, document services promptly.
