Job summary

Qualifications

Responsibilities

• Evaluate patients for hospice eligibility and plan nursing care.
• Provide nursing care to patients and families in all settings.
• Document patient encounters accurately and timely in records.
• Maintain patient confidentiality and support patient rights.
• Coordinate with families and healthcare team to support goals.
