Job summary

University of Vermont Health is seeking a Registered Nurse for Visit/Resource Nurse - Community Hospice to join our homecare team. You will deliver skilled nursing care in patients' homes, focusing on education and palliative support for both patients and families.

The role requires Vermont RN licensure, with a strong preference for hospice/palliative care certification and prior home health experience. A per diem schedule with orientation and ongoing education is offered.

Qualifications

• Associate's degree in nursing or higher is required.
• Minimum of one or more years of directly related home or community hospice nursing experience OR two or more years of related home or public health nursing experience OR three or more years of general nursing experience, including at least one year of directly related home/public health or hospice nursing experience.
• Current RN licensure recognized by the State of Vermont is required; Hospice and palliative care certification preferred.

Responsibilities

• Provide full range of skilled nursing care to patients in homecare/home hospice setting with focus on patient education and palliative care.
• Educate and support families in managing symptoms and care at home.
• Collaborate with interdisciplinary team to coordinate care and ensure patient comfort and safety.
