CommuniCare Health invites applications for a Hospice RN Field Nurse. This role plans, organizes and directs home care services with emphasis on community health education, delivering patient-centered care in the home environment.

You will assess patients, supervise LPNs and aides, develop care plans, and administer medications as prescribed while coordinating discharge planning and acting as Case Manager for an assigned caseload.

Formación

• Graduate of an accredited school of nursing.
• Active RN licensure.
• 1–2 years of recent acute care experience in an institutional setting.
• Bachelor's degree with 1 year home health care experience preferred.
• Must have a valid driver's license with an insured vehicle.

Responsabilidades

• Plan, organize, and direct home care services with emphasis on community health education.
• Complete initial patient assessments and develop care plans.
• Supervise LPNs and Home Health Aides implementing the plan of care.
• Administer medications and treatments as prescribed.
• Coordinate discharge planning and act as Case Manager for assigned caseload.
