Job summary

ATC & CareBuilders - Parent Account is seeking a Hospice RN- Field in Florida. The role covers care for patients in private residences, assisted living facilities, and skilled nursing homes, with supervision of LPNs and CNAs.

The RN field nurse will educate, assess, and intervene to maximize comfort and quality of life for patients and families. Reporting to the Clinical Care Manager, this PRN position requires Florida RN licensure, strong ethics, and reliable transportation.

Qualifications

• Florida RN license required.
• Minimum 1 year nursing experience in hospital, home health or hospice preferred.
• Strong ethics and professional demeanor.
• Sound critical thinking and time management skills.
• Well-organized, self-directed team player.
• Reliable transportation and valid Florida driver’s license.

Responsibilities

• Case manages patient/family needs in the field.
• Performs ongoing assessment and intervenes as needed.
• Oversees supervision of LPNs, CNAs, and other staff.
• Identifies problems and supports policies and procedures.
• Mentors and guides coworkers; maintains interdisciplinary relationships.
• Completes documentation prior to end of shift.
• Participates in IDG and communicates patient needs to team.
• Collaborates with the team physician daily.
• Performs other duties as requested by the Nurse Care Manager.
