Qualifications

• Current unencumbered Ohio RN license.
• One year of acute care or medical-surgical nursing experience.
• Experience in Medicare-certified home health preferred.
• Proficiency with Windows, MS Office, and EMR.
• Must read, write, speak, and understand English.

Responsibilities

• Care management and service delivery in patients' homes or facilities.
• Develop and implement comprehensive care plans.
• Provide pain and symptom management.
• Communicate with other health care providers about changes in condition.
• Assist with IDG prep by reviewing Care Plans and medications.
