Job summary

Ohio Living is seeking an RN Field Staff to plan, coordinate and deliver high-quality home health and hospice care in assigned communities. You will assess patient needs, develop care plans, and communicate with the care team to adjust plans as conditions change.

Qualifications include an Ohio RN license, at least one year of acute care experience, and proficiency with EMR, Windows and MS Office. The role requires driving and flexible hours to meet patient needs, including on-call availability.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
• One year experience in acute care or medical-surgical nursing required.
• Experience in a Medicare-certified home health agency preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

Responsibilities

• Care Management and Service Delivery - Provides ongoing assessment and nursing care to patient in homes and health care facilities; develops and implements a comprehensive care plan.
• Quality and Compliance Management - Audits and maintains patient records and ensures regulatory compliance.
• People Management and Development - Coaches and guides field staff to meet performance expectations and ensures staff training.
