Job summary

VitalCaring Hospice RN role focuses on delivering comfort, dignity, and compassionate support in field-based settings for end-of-life care. You will lead plans of care, manage symptoms, and coordinate with physicians, social workers, chaplains, and aides to ensure individualized, comfort-focused care.

You’ll provide patient and family education, support decision-making with empathy, and maintain accurate documentation while collaborating with the IDT for quality and compliance.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care and develop comfort-focused care plans.
• Serve as care coordinator and case manager across the interdisciplinary team.
• Provide education and emotional support to patients and families.
• Maintain documentation and participate in care planning and quality/compliance processes.
