VitalCaring seeks a Hospice Registered Nurse (RN) to lead patient-centered hospice care in home and community settings. You will assess, develop comfort-focused plans, and coordinate skilled nursing visits with empathy and clinical excellence.

This field-based role offers high autonomy with team support, on-call weekend schedule (Thu–Mon) in Metairie/Covington, LA area, requiring hospice experience and ability to manage a caseload while communicating compassionately with patients and families.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Lead Patient-Centered Hospice Care: conduct comprehensive patient and family assessments; develop and manage individualized, comfort-focused care plans; deliver skilled nursing care with focus on pain and symptom management.
• Serve as the Care Coordinator: act as case manager, coordinating care across the interdisciplinary team; collaborate with physicians, social workers, chaplains, and aides; monitor patient condition and adjust plan for comfort.
• Support Patients & Families: provide education, emotional support, and guidance; help families navigate decisions with empathy; build trusting relationships.
• Drive Quality & Compliance: maintain accurate documentation; participate in IDT meetings and care planning; support standards.
