Job summary

VitalCaring – Deliver Care That Brings Comfort and Dignity. VitalCaring Hospice RN role is field-based, leading plans of care and coordinating with physicians, social workers, chaplains, and aides to provide comfort-focused, patient-centered end-of-life care.

You will assess patients and families, manage symptoms, and advocate for goals and wishes, delivering reassurance and clear guidance in sensitive situations. Field-based care emphasizes autonomy with team support.

Qualifications

• Active RN license (or compact eligibility) required.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care, developing and managing comfort-focused plans.
• Serve as care coordinator across the interdisciplinary team.
• Support patients and families with education and guidance.
• Drive quality and compliance with documentation and care planning.
