Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered end-of-life care in homes and facilities, delivering comfort and dignity. You will assess patients, manage symptoms, and coordinate with physicians, social workers, chaplains, and aides to ensure goals and wishes are met with compassion.

You will guide care plans, support families, and navigate complex decisions with empathy, while maintaining rigorous documentation and compliance in a fast-paced field-based role.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care, developing and managing comfort-focused plans.
• Serve as the care coordinator across the interdisciplinary team.
• Support patients and families with education and empathetic guidance.
• Drive quality and compliance through accurate documentation and care planning.
