Job summary

Hospice of Marion, a Part of Empath Health, is seeking an RN (PRN) to join our team for after-hours patient visits in homes, long-term care, and assisted living facilities in Marion County. The role includes evening and overnight shifts, with 2 weekend 12-hour shifts per month.

As part of Empath Health, we provide Full Life Care and compassionate end-of-life support. The RN will supervise LPNs and HHAs, educate IDT members, and ensure medication administration and patient care are performed per

Qualifications

• Ability to obtain patient history and perform age-specific assessments.
• Documentation compliant with organizational standards.
• Educate IDT members on nursing aspects of patient/family care.
• Administer, monitor and document medications as prescribed.

Responsibilities

• Obtain patient history and perform physical, psychosocial and spiritual assessments.
• Collaborate with IDT, physician and facilities for appropriate referrals and interventions.
• Provide education to patients, families and IDT on nursing care.
• Supervise Home Health Aide and LPN; evaluate services biweekly.
