Hospice of Michigan in Big Rapids, MI seeks a Registered Nurse to deliver professional nursing services and supervise care for hospice patients per standards and guidelines.

The RN will perform assessments, develop and update care plans with the IDT, provide end-of-life care and educate families, while ensuring compliance with regulatory requirements; CHPN preferred.

Candidates must have MI RN license and be able to work variable hours, including weekends.

Competenze

• Graduation from an accredited nursing program with MI RN license.
• 1–2 years nursing experience; hospice preferred.
• Proficient with clinical technology and documentation tools.
• Excellent communication, organizational skills and ability to maintain confidentiality.
• Ability to work variable hours, including weekends.

Mansioni

• Performs complete physical assessments and histories of illnesses.
• Develops, implements, evaluates and updates the individualized plan of care with revisions as needed.
• Collaborates with the IDT to provide patient/family hospice services.
• Provides end-of-life care and symptom management.
• Educates patients and families on disease, self-care, end-of-life care and ethical considerations.
• Coordinates ordering and delivery of durable medical equipment and supplies.
• Documents care and communicates with physicians and the care team.
• Supervises Hospice Aides and ensures regulatory compliance.
• Participates in quality assurance and process improvement activities.
• Adheres to CARE model and regulatory requirements.
