Job summary

Chapters Health System seeks an RN for Day Triage to handle telephonic inquiries from patients and families, delivering compassionate care and accurate information. You will enter data into the EMR, coordinate referrals, and support in-person visits as needed.

The role requires an active RN license, 1 year nursing experience (hospice/hospital preferred), BLS certification, and strong communication skills in English; Spanish is a plus.

Qualifications

• Active RN license in one state with ability to obtain multi-state license within 30 days
• Minimum 1 year nursing experience; hospice or hospital preferred
• BLS for healthcare professionals (AHA/Red Cross)
• Experience with EMR/EHR systems
• English proficiency; Spanish preferred

Responsibilities

• Triage phone calls and ensure callers' needs are met with excellent service
• Provide direct care to patients and families with psychosocial, spiritual, cultural and physical components
• Document patient information accurately in the EMR and coordinate with IDG
• Resolve inquiries and arrange in-person visits when needed
• Support continuity of care during emergencies and adverse events
