Job summary

Qualifications

• Current RN licensure and graduation from an approved nursing program.
• At least two years of clinical nursing experience, with one year preferred in hospice care.
• Knowledge of home health and hospice operations and the nurse’s role within the care team.
• Understanding of nursing practices, agency policies, and reimbursement guidelines.
• 12 hours of continuing education annually and CPR certification.

Responsibilities

• Apply nursing theories and concepts to patient care.
• Document care in accordance with reimbursement guidelines.
• Coordinate physical care with patients, families, physicians, and the hospice team.
• Communicate effectively with patients, families, and agency personnel.
• Participate in interdisciplinary team meetings and quality reviews.
