Job summary

Agape Hospice Care is seeking a compassionate Registered Nurse to administer skilled nursing care for clients in their homes, coordinating with the interdisciplinary team and families to deliver high-quality hospice services.

Required: an accredited nursing degree (Associate or Bachelor’s), current RN license, CPR certification, and experience in clinical care; hospice or home health experience is preferred. Strong communication, interpersonal, and organizational skills are essential for success.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and a valid driver’s license.
• Two years of RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification and proficiency with computer use.

Responsibilities

• Assess patients' physical, psychosocial, and environmental needs and plan care.
• Coordinate care with IDT, patient, family, and referring agency.
• Provide on-call coverage and document progress notes.
• Deliver nursing procedures and patient education as needed.
• Evaluate and adjust care plans based on patient needs.
