Job summary

Hospice Registered Nurse (RN) in Plymouth, NH, provides direct client care and case management for a caseload in home settings. Duties include admission, developing a care plan, and ongoing evaluation with the physician and multidisciplinary team. Licensed in NH or a compact state is required.

The role involves field visits, 37.5 hours per week, and occasional on-call. Compensation includes hourly pay with mileage reimbursement and full benefits.

Qualifications

• Hospice experience preferred or 1 year in med/surg/ER/home health if no Hospice experience.
• State of NH or Compact Nursing License required.
• Minimum ADN preparation.
• NH Driver’s License and automobile liability insurance.
• Strong documentation and communication skills.
• Comfortable with death and terminal illness.

Responsibilities

• Provide direct client care and case management in home setting.
• Develop and implement plan of care with physician orders.
• Ensure timely and accurate documentation and care plans.
• Communicate with multidisciplinary team and community resources.
• Manage an assigned caseload and ensure reimbursement.
