Job summary

BAYADA Hospice is seeking a compassionate, full-time Registered Nurse (RN) to join our hospice office in Concord, NH. The role involves making home visits to clients in Merrimack, Belknap and Hillsborough counties, with opportunities to service New London and Warner areas.

Prior home care or hospice experience is preferred. You will be part of a multidisciplinary team providing palliative care to help clients remain at home.

Qualifications

• One year of recent, verifiable clinical experience preferred but not required.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to Clinical or Client Services Manager.
• Follow up with and document doctors\' orders.
• Perform client assessments as necessary.
• Occasional on-call after hours and/or weekends.
