Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse (RN) to provide compassionate care for hospice patients in home and facility settings. You will handle admissions, patient visits, and symptom management, while communicating with families and care teams.

The role includes after-hours on-call shifts and requires driving across the service area with reliable transportation. Qualified candidates hold an active RN license, prefer admissions and on-call experience, and excel in team

Qualifications

• Current Registered Nurse licensure (in good standing) in state of practice with 1 year of experience
• Admissions and On-call experience preferred
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
• Works closely with patients on a 1-1 level with support from a patient care team
• Ability to work independently with day-to-day activities including phone, email and written communication and meetings
• Time management skills to manage visits, documentation, and meetings
• Ability to learn an electronic medical records system
• Willingness to drive with reliable transportation and travel across the service area

Responsibilities

• Provide care and services for hospice patients in homes, facilities or hospitals
• Complete hospice admissions and patient visits as needed
• Answer calls from patients, families and customers
• Make nursing visits for symptom management, deaths and admissions
• Be a passionate advocate for hospice services and Suncrest Hospice
• This position will be after hours on-call, seven days/nights on and seven nights/days off
