Job summary

Brighton Hospice is seeking an experienced Registered Nurse to provide compassionate hospice care for patients in their homes and facilities.

You will conduct admissions and patient visits, respond to calls from patients and families, and collaborate with a care team. After-hours on-call rotates seven days on/off, with travel across the service area required. This role supports a CHAP-certified hospice striving for exceptional patient care.

Qualifications

• Current RN licensure (in good standing) in the state(s) of practice.
• Admissions and On-call experience preferred.
• Works well within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to learn an electronic medical records system.
• Must be willing to travel across the service area with reliable transportation.

Responsibilities

• Provide care and services for hospice patients, visiting patients where they reside (homes, facilities, hospitals).
• Complete hospice admissions and patient visits as needed.
• Answer calls from patients, families and customers.
• Perform nursing visits for symptom management, admissions, and deaths as needed.
• After hours on-call seven days/nights on and seven nights/days off.
