Job summary

Enhabit Home Health & Hospice in Atlanta/Georgia region is seeking a dedicated Registered Nurse for home health and hospice services. You will coordinate care, assess patient and family needs, and implement the interdisciplinary plan of care in alignment with physician orders.

The role requires an RN license, at least 1 year of RN experience, and comfort with mobile technology. Home health experience is preferred; the department offers support and development opportunities.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient’s and family’s needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.
