Job summary

Affinis Hospice in Augusta, GA is seeking a registered nurse to provide professional care to hospice patients, following physician orders and the plan of care.

This role emphasizes pain and symptom management, collaboration with the interdisciplinary team, timely documentation, and support for patients and families through the dying process. A valid GA RN license and CPR are required.

Qualifications

• Graduate from an accredited School of Nursing.
• Currently licensed as an RN in Georgia.
• Minimum six months of acute care RN experience; over one year preferred.
• Current CPR certification.
• Able to meet health clearance and provide professional references.

Responsibilities

• Assumes primary responsibility for a Hospice Services patient/family caseload and nursing process phases.
• Initiates communication with physicians and interdisciplinary teams to coordinate care.
• Assess patients within 48 hours of admission and support comprehensive assessment within 5 days.
• Maintain up-to-date patient records reflecting care plans and changes.
• Collaborate in IDG conferences and provide on-call coverage as needed.
• Support patients and families through the dying process with dignity and compassion.
