Affinis Hospice is hiring a Licensed Practical Nurse to provide compassionate home-based nursing services in accordance with the Plan of Care and physician orders. You will document care activities accurately, support patients and families, and collaborate with the interdisciplinary team to maintain comfort and dignity.

The role requires current LPN license, CPR certification, and the ability to travel to patient homes across Georgia.

Formación

• Must hold a nursing degree or be a graduate of an approved LPN program.
• Current license/registration by state of hire as an LPN.
• Current CPR certification.

Responsabilidades

• Provides direct patient care as defined in the LPN State Nurse Practice Act.
• Implements current nursing practice following a patient assessment and approved Plan of Care.
• Documents Hospice patient services accurately to reflect the Plan of Care.
• Educates patients, families and caregivers about diagnosis and Plan of Care.
• Coordinates agency services and reports needs to Interdisciplinary Group members.
• Uses equipment and supplies effectively and efficiently.
• Manages pain and symptoms; evaluates responses to treatments.
• Participates in professional growth and quality management programs.
• Represents the System with servant leadership and clear communication.
• Contributes to committees and project work to support system goals.
• Performs other duties to ensure program success.
