Job summary

Affinis Hospice in Georgia is seeking a licensed practical nurse to provide compassionate nursing services in patient homes or facilities, following the Plan of Care and physician orders.

You will document care accurately, educate patients and families, coordinate with the interdisciplinary team, and participate in quality management to ensure the highest level of comfort and dignity for hospice patients.

Qualifications

• Nursing degree from an accredited college or university or is a graduate of an approved LPN program.
• Current license / registration by state of hire as a LPN.
• Current CPR certification.

Responsibilities

• Provides direct patient care as defined in the LPN State Nurse Practice Act.
• Implements current nursing practice following a patient assessment and an approved Plan of Care initiated by the Hospice Services Registered Nurse.
• Provides accurate and timely documentation of Hospice patient services to reflect the Plan of Care.
• Assesses and provides Hospice Services patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
• Participates in coordination of agency services, appropriately reporting the identified needs for agency Interdisciplinary Group members.
• Uses equipment and supplies effectively and efficiently.
• Provides appropriate pain/symptom management; evaluates Hospice Services patient’s response(s) to treatments/medications.
• Participates in personal and professional growth and development. Also participates in the organization’s quality management program.
• Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
• Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
• Performs other duties as necessary to ensure the success of the System.
