Job summary

Avanta Care Home Care - Atlanta is seeking a Hospice Registered Nurse (RN) to provide skilled nursing in home and facility settings. The RN coordinates patient care, ensuring comfort, dignity, and quality of life for terminally ill patients and their families.

The role requires current RN licensure in GA and at least 1 year of clinical nursing experience, ideally in hospice or home health, plus strong communication and critical thinking skills and a valid driver's license.

Qualifications

• Current RN license (State of Georgia).
• Minimum 1 year of clinical nursing experience (hospice or home health preferred).
• Excellent communication, assessment, and critical thinking skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments for pain and symptom management.
• Educate patients and families on disease process, care techniques, and expectations.
• Collaborate with physicians, social workers, chaplains, and volunteers.
• Document all care accurately in EMR according to regulatory standards.
• Participate in interdisciplinary team (IDT) meetings and case conferences.
• Provide emotional support and crisis intervention for patients and families.
• Ensure compliance with Medicare, state, and agency policies.
