Job summary

Agape Hospice Care Atlanta is seeking a compassionate RN to deliver skilled nursing services in patients' homes across Cobb, Fulton, Clayton, Henry, and Fayette Counties, with on-site interviews at our Villa Rica office. You will coordinate with the interdisciplinary team, educate families, and manage care plans for patients of all ages in hospice care.

The role requires an accredited nursing degree, an active RN license, CPR certification, and at least two years of clinical RN experience; home

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills; high ethical standards.

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments in patients' homes.
• Coordinate care with the interdisciplinary team (IDT) and family; manage the care plan.
• Educate patients and families on disease process, end-of-life care, and self-care techniques.
• Provide on-call nursing coverage as assigned.
• Document progress notes and adapt plans as patient needs evolve.
• Assess caregiver support and adapt services to acuity and needs.
