Job summary

Qualifications

• Active Illinois RN license (IDFPR).
• Current CPR certification.
• 2–3 years hospice nursing experience, with leadership exposure.

Responsibilities

• Provide clinical supervision to hospice RNs, LPNs, and CNAs.
• Lead quality assessments and quality improvement initiatives.
• Develop and implement individualized patient care plans with the interdisciplinary team.
• Perform home visits to assess patient needs and provide education and support to patients and families.
• Participate in development and implementation of hospice programs and services.
• Communicate effectively with patients, families, and other healthcare professionals to ensure comprehensive care.
• Maintain accurate and timely documentation of patient care and outcomes.
