Hospice RN Clinical Supervisor

Job Summary:

Gemini Hospice, a leading provider of compassionate healthcare services, is seeking a dedicated and experienced Hospice Agency Clinical Supervisor to join our team in Morton Grove, IL. As a key member of our organization, you will provide clinical oversight, leadership, and regulatory compliance for the hospice interdisciplinary team. This role ensures high quality, patient-centered end of life care through supervision, coaching, case review and adherence to state and federal hospice standards. The Clinical Supervisor supports staff, monitors performance, and ensures that care plans reflect the patient's goals, comfort, and dignity.

Responsibilities:

• Provide clinical supervision and guidance to hospice RNs, LPNs, and CNAs to ensure compliance with regulatory requirements and hospice policies and procedures
• Conduct regular quality assessments and quality improvement initiatives to ensure high-quality patient care
• Collaborate with the interdisciplinary team to develop and implement individualized patient care plans
• Conduct home visits to assess patient needs and provide education and support to patients and families
• Participate in the development and implementation of hospice programs and services
• Communicate effectively with patients, families, and other healthcare professionals to ensure comprehensive care
• Maintain accurate and timely documentation of patient care and outcomes

Requirements:

• Active Illinois RN license in good standing (IDFPR)
• Current CPR certification
• Valid Driver's license and reliable transportation
• Minimum of 2-3 years of experience in hospice nursing, with a strong preference for experience in clinical supervision or prior leadership roles, charge nurse
• Strong knowledge of CMS Hospice Conditions of Participation and IDPH Hospice Regulations
• Ability to work flexible hours, including evenings, weekends, and holidays (if required)
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Familiarity with hospice regulations and guidelines
