Job summary

BAYADA HOME HEALTH CARE in Wyomissing, PA seeks a full-time Clinical Manager (RN) – Hospice to lead a multidisciplinary field team providing compassionate end‑of‑life care across Berks, Lehigh, and Northampton counties.

You will oversee clinical care planning, review referrals, manage caseloads, and lead IDG meetings. The role requires an RN license in PA, hospice experience, and strong leadership and communication skills to support staff and ensure service excellence.

Qualifications

• Current RN license in Pennsylvania in good standing.
• Graduate of an accredited nursing program; BSN preferred where applicable.
• Hospice experience and leadership experience.
• Strong communication, interpersonal and problem-solving skills.
• Ability to lead with initiative, adaptability and sound clinical judgment.
• Comfort leading a multidisciplinary field team.

Responsibilities

• Lead and support field staff including RNs, CNAs and home health aides to ensure high‑quality hospice care.
• Respond to and help manage clinical calls from nurses in the field.
• Review referrals and hospital notes to support safe care planning.
• Lead IDG meetings and coordinate care with the interdisciplinary team.
• Oversee caseloads and adapt to admissions and discharges in a dynamic hospice setting.
• Provide proactive leadership to support staff and ensure strong service delivery.
• Collaborate with office and clinical leaders to align team efforts.
