Job summary

St. Camillus is seeking an experienced Hospice Clinical Care Manager to coordinate the day-to-day operations of our Hospice Agency and help ensure the delivery of quality, comprehensive care in accordance with federal, state, Joint Commission, and St.

Camillus guidelines. The Hospice Clinical Care Manager will lead, coach, and support clinical staff, coordinate admissions and nursing visits, monitor quality and documentation, and serve as a liaison with staff, physicians, families, and campus

Qualifications

• Current Wisconsin RN license and current CPR certification.
• Minimum of five years of hospice experience.

Responsibilities

• Lead, coach, and support Hospice clinical staff.
• Coordinate admissions, daily nursing visits, case conferences, and care services.
• Monitor quality outcomes, documentation, compliance, and reporting practices.
• Serve as a resource and liaison for staff, physicians, allied health professionals, families, and campus partners.
