The Mount Carmel Hospice RN Case Manager position in Columbus, OH, is a full-time day shift role within Trinity Health's Mount Carmel Home Care & Hospice. You will coordinate and manage care for hospice patients and families, developing individualized care plans and leading an interdisciplinary team to ensure quality outcomes.

Responsibilities include comprehensive assessments, pain and symptom management, patient and family education, and ongoing documentation.

Qualifications

• Graduate of an accredited nursing program.
• Current Ohio RN license.
• CPR certification.
• Minimum one year of acute care nursing experience.
• Valid drivers license and reliable transportation.
• Preferred hospice or home health experience.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Coordinate care with physicians, social workers, chaplains, aides, and other team members.
• Manage pain and symptoms using evidence-based hospice practices.
• Provide education and support to patients, families, and caregivers regarding disease progression and end-of-life care.
• Monitor patient status and communicate changes to the interdisciplinary team.
• Supervise Hospice Aides and LPNs to ensure quality care delivery.
