PruittHealth Hospice is hiring a Registered Nurse Case Manager to provide in-home or facility-based hospice care in the North Wilkesboro area. This full-time role supports patients and families with compassionate nursing care, assessment, and care coordination.

The successful candidate will maintain clinical accuracy, adhere to regulatory guidelines, and collaborate with a multidisciplinary team to ensure continuity of care for our patients and their loved ones.

Qualifications

• Current, active RN license in North Carolina.
• Experience in hospice or home health is preferred.
• Strong clinical assessment and documentation skills.

Responsibilities

• Conduct evaluations, assessments, and treatments per care plan.
• Complete timely documentation and records.
• Adhere to policies and maintain continuity of patient care in hospice.
